Successful bidder(s) shall furnish an approved performance bond, labor and material payment bond, each in the amount of one hundred percent (100%) of the contract amount. Insurance as required shall be obtained by the successful bidder(s) and a certificate of that insurance shall be provided.
The right is reserved to reject any or all proposals received, to waive informalities, to postpone the award of the contract for a period of not to exceed thirty (30) days, and to accept the bid which is in the best interests of the Upper Green River Cemetery District.

Jerry Jensen, president, chaired this meeting held in the Sublette County Commissioners Room, Pinedale, Wyo. Mary Lynn Worl, vice president, Bill Barney, secretary/treasurer, and Garry Eiden, trustee, were in attendance. Walt Bousman, trustee, was absent. Jauna McGinnis, clerk, took the minutes.

CALL TO ORDER
Jerry Jensen called the meeting to order at 7 p.m. and set the agenda. Mary Lynn Worl motioned to accept the agenda as read. Motion seconded by Bill Barney and carried unanimously.
MINUTES/PHYSICIAN CHECKS
Bill Barney motioned to accept the Dec. 30, Jan. 6, 13, 17, 20 and 28 special and Jan. 10 regular meeting minutes as presented. Motion seconded by Mary Lynn Worl and carried with Garry Eiden abstaining from approving the special meeting minutes.
After reviewing the physician checks, Mary Lynn Worl motioned to approve the physician's checks as presented. Motion seconded by Garry Eiden and carried unanimously.
PHYSICIAN BUSINESS
DR. BURNETT ADDRESS
Dr. Burnett read a letter terminating his contract as of May 26, 2005.
The board thanked him for his service and wished him well.
HELIPORT
No update available.
BUILDING MAINTENANCE UPDATE
The board re-addressed this issue. Garry Eiden reported contractors he has spoken with are not interested. Bill Barney motioned to advertise for building maintenance contracts in both papers. Motion seconded by Garry Eiden and carried unanimously.
EMS BUSINESS
BACKGROUND CHECKS/DRUG TESTING
Mary Lynn Worl reported a policy and procedure needs to be in place. She obtained a sample copy to help write a policy. Feedback will be obtained from legal counsel.
EMT SALARIES
The progress of the analysis was presented. A tier system has been designed and different options were discussed. Bill Barney motioned to adopt this proposal with a sunset clause to resolve in the following budget session. Motion died for lack of a second.
Discussion turned to a delayed ambulance response time. The board concluded the reason for the delay was no call was made to 911.
Payment for the supervisor position was briefly discussed. Jerry Jensen reiterated a $300 stipend was originally set up by the board for each supervisor. The group decided to split the stipend amongst their executive committee members.
Bill Barney motioned that the board and two representatives from each EMS division come up with a mutually agreeable salary structure with a mutually agreed meeting date when information is refined by both the board and EMS. Motion seconded by Mary Lynn Worl and carried unanimously. A copy of the tier system will be sent to each EMS division.
BIG PINEY AMBULANCE
The new ambulance is expected in March.
CERTIFICATE OF OCCUPANCY
Jauna McGinnis reported the occupancy has been approved by the state fire marshal.
HOMELAND SECURITY FUNDS
Jerry Jensen reported he spoke with Bob Hanson who did not know anything about funds for EMTs to attend the oilfield class. The funds were available to the sheriff department to hire one or two deputies to patrol the Jonah Field area. Wil Gay will try to submit radios through this fund. He will also submit an outline of information of what is available and to whom.
BONDURANT BEC CLASS
Wil Gay reported he is conducting the class once a week. It will end sometime in April. There are 18 attending; 7 from Big Piney, 10 from Bondurant, 1 from Pinedale. He would like to operate the BECs under Pinedale EMS.
OTHER
Jerry Jensen indicated that citizens need to call the 911 system, not the ambulance barn. Wil Gay will ensure that Dispatch will call Big Piney if no ambulance is out in five minutes from Pinedale. Each EMS division will deal with the incoming calls requesting an ambulance. It was questioned whether or not the EMS service could set-off their own tone.
FIRST AID INSTRUCTORS
Two EMTs from each EMS division would like to attend the First Aid Instructor course. Bill Barney motioned for the Pinedale and Big Piney EMTs to attend the course. Motion seconded by Garry Eiden and carried unanimously.
STANDBY ISSUES
Questions have been raised regarding standby's. Jerry Jensen read the policy for clarification and asked the EMTs to be gracious to the other service in the area who is specifically set-up to provide service for forest fires and stand-bys.
NEW HIRE UPDATE/EMT ADVERTISEMENT
David Potter is waiting for a call pertaining to housing. The board will keep advertising until the second position is filled. Applications have been sent to five other individuals.
GENERAL BUSINESS
The meeting was recessed at 8:55 p.m. Jerry Jensen brought the meeting back to order at 9:11 p.m.
DIRECTOR/LAND ACQUISITION
The board retired into executive session at 9:15 p.m. to discuss the director position and land acquisition issues.
Jerry Jensen brought the meeting back to order at 9:59 p.m. Mary Lynn Worl motioned to offer the job to Randal Johnson with a starting salary of $67,500 increasing to $70,000 after a six month favorable review. In addition moving expenses will be paid. Motion seconded by Bill Barney and carried with the vote being Mary Lynn Worl and Bill Barney in favor, Garry Eiden opposed. Bill Barney motioned to amend the moving expenses to be approved by the board. Motion seconded by Mary Lynn Worl and carried with the vote being Bill Barney and Mary Lynn Worl in favor, Garry Eiden opposed.
Mary Lynn Worl will ask to release his information to the press upon acceptance.
BILLING SOFTWARE UPGRADE
Jauna McGinnis reported the billing system has quit working. Mary Lynn Worl motioned to purchase the new billing system. Motion second ed by Bill Barney and carried with the vote being Mary Lynn Worl and Bill Barney in favor, Garry Eiden opposed. The board will look at the trip report module at a later date.
EMS CALL SCHEDULE (PINEDALE)
Jacque Strike is willing to cover morning shifts Monday through Thursday to ensure two medics for each shift as long as it fulfills her hourly requirement. Mary Lynn Worl motioned to have Jacque Strike cover the morning shifts and be paid for mileage on the two extra days. Motion seconded by Garry Eiden and carried unanimously.
AUDITOR
Jauna McGinnis reported the auditor used by Sublette County is willing to incorporate the district into their schedule. Other companies indicated they were too busy. Bill Barney motioned for Leo Riley to send a letter of engagement. Motion seconded by Mary Lynn Worl and carried unanimously.
MEDICARE
Jauna McGinnis reported that Medicare beneficiaries in the county are experiencing ambulance denials. Many have called and asked if there is anything they can do. Roy Snow of Big Piney initiated a call to Barbara Cubin's office, who in turn called the district office. They are in the process of trying to change the regulation. They will keep in contact with the office.
CD ROLLOVER
The CD has matured. Mary Lynn Worl motioned to roll over the CD at the Bank of Pinedale. Motion seconded by Bill Barney and carried unanimously.
EQUIPMENT DISPOSAL
Jauna McGinnis requested permission to dispose of a typewriter. Mary Lynn Worl motioned to dispose of the typewriter. Motion seconded by Garry Eiden and carried unanimously.
OTHER
Mary Lynn Worl motioned to advertise for physicians. Motion seconded by Bill Barney and carried unanimously.
TABLED ITEMS
The board tabled community donations, conference calling update, Bill Schertz, and policy and procedure meeting discussion.
DISBURSEMENT APPROVAL
The check registers were reviewed for accountability of checks. No discrepancies were noted. Garry Eiden motioned to pay the bills as presented. Motion seconded by Bill Barney and carried unanimously authorizing vouchers for payroll and vendors numbered 4545 through 4639, DD5043-10 through DD5043-31, DD5055-1 through DD5055-3, EFT5045-2069, and EFT5056-200, totaling $181,912.02.
ADJOURNMENT
Without any further business, Mary Lynn Worl motioned to adjourn the meeting. Motion seconded by Bill Barney and carried unanimously. Meeting adjourned 10:45 p.m.

BIDS will be received by Rio Verde Engineering, at their office, until 2:00 P.M., local time on March 16, 2005, at which time said BIDS will be publicly opened and read aloud.
The work consists of constructing 1200 lineal feet of town street including 1730 feet of 3' valley pan, 130 feet of 6' valley pan, 300 feet of 18" CMP, 3610 feet of utility trenching, miscellaneous concrete, street signs, constructing one highway approach onto U.S. 191, and preparing crushed base for chip seal.
Copies of the Project Manual and the Design Drawings specifying the work to be performed may be obtained from Rio Verde Engineering.
BIDDERS shall be required to accompany each bid with a BID Bond with sufficient surety, or a certified check, cashier's check or bank draft upon some reputable bank payable to LWI Investments, Inc. for at least ten percent of the total amount of the BID. The bond, check or bank draft shall be forfeited as liquidated damages, if the BIDDER, upon the letting of the contract to him, fails to enter into the contract within ten days after it is presented to him for that purpose or fails to proceed with the performance of the contract. The BID Bond, draft or check shall be retained by LWI Investments, Inc., until proper bond to secure performance of the contract has been filed and approved. All the above is to be in accordance with 15-1-113, Wyoming Statutes, 1977, July 1980 Supplement, or as amended.
The right is reserved by LWI Investments, Inc. to reject any and all BIDS, and to waive any irregularity or informality in the bidding. BIDS may not be withdrawn for thirty days after the date of opening.

CALL FOR BIDS
The Town of Pinedale is calling for bids on the cleaning of the Town Hall. Contract, including specifications of services required are available at the Town Hall, 210 West Pine Street, Pinedale, Wyoming, during regular working hours. Interested persons or firms are invited to submit sealed bids no later than 5 p.m., Monday, March 14th, 2005. Bids may be dropped off at the Town Hall or may be mailed to the Town of Pinedale, P.O. Box 709, Pinedale, Wyoming 82941. Bids should be on a form provided by the Town Clerk or a similar document and should be accompanied by a list of references. All bids shall be in sealed envelopes bearing the words "Sealed Bid-Office Cleaning".

ORDINANCE NO. 368
AN ORDINANCE ANNEXING ADJACENT AND CONTIGUOUS LAND (WRANGLER ANNEXATION) TO THE TOWN OF PINEDALE, WYOMING, AND ESTABLISHING ZONING THEREFOR.
WHEREAS, all the owners of the real estate described herein, excepting certain lands on which is located a Wyoming State Highway, have signed and filed with the Town a petition requesting annexation and zoning of said real estate; and
WHEREAS, after notice and hearing as required by law, the Town Council has, by resolution duly adopted on 2004-8 found and certified that the conditions required by W.S. Sections 15-1-402 and 15-1-403 exist, and that annexation of the real estate is authorized by law;
BE IT ORDAINED BY THE TOWN COUNCIL OF THE TOWN OF PINEDALE:
SECTION 1. The following described real estate located in Sublette County, Wyoming, is hereby added to, taken into, annexed to and made a part of the Town of Pinedale, Wyoming, to-wit:
Lot Two (2) (NW1/4NE1/4), Section 5, Township 33 North, Range 109 West of the 6th P.M., Sublette County, Wyoming being more particularly described as Tract Two (2) of the Bloom Lot Division as filed in the Office of the Clerk of Sublette County, Wyoming in Book 70 of Misc., Page 592 and the north 70 feet of said Lot Two (2) (NW1/4NE1/4), and that said Wrangler Annexation contains 37.13 acres, more or less;
SECTION 2. Upon the effective date of this ordinance, the corporate limits and boundary lines of the Town of Pinedale, Wyoming, shall be extended to include and embrace the property described in Section 1.
SECTION 3. The annexed property is hereby zoned C-1.
SECTION 4. The zoning map is hereby amended to reflect the zoning classification of the annexed property.
This Ordinance shall be in full force and effect from and after passage on three readings and publication according to law.

We're pleased to present to you this year's Annual Quality Water Report. This report is designed to inform you about the quality water and services we deliver to you every day. Our constant goal is to provide you with a safe and dependable supply of drinking water. We want you to understand the efforts we make to continually improve the water treatment process and protect our water resources. We are committed to ensuring the quality of your water. Our water source consists of surface water drawn from Fremont Lake.
We are pleased to report to our consumers that our drinking water is safe and meets federal and state requirements.
If you have any questions about this report or concerning your water utility, please contact the Ron Brown or Randy Hubburd at 307-367-4136. We want our valued customers to be informed about their water utility. If you want to learn more, please attend any of our regularly scheduled meetings. They are held on the second and fourth Monday of every month at 6:30 p.m., located at the Town Hall.
Town of Pinedale routinely monitors for constituents in your drinking water according to federal and state laws. This table shows the results of our monitoring for the period of Jan. 1 to Dec. 31, 2004. As water travels over the land or underground it can pick up substances or contaminants such as microbes, inorganic and organic chemicals and radioactive substances. All drinking water, including bottled drinking water, may be reasonably expected to contain at least small amounts of some constituents. It's important to remember that the presence of these constituents does not necessarily pose a health risk. More information about contaminants and potential health effects can be obtained by calling the Environmental Protection Agency's Safe Drinking Water Hotline at 1-800-426-4791.
In this table you will find many terms and abbreviations you might not be familiar with. To help you better understand these terms we've provided the following definitions:
Parts per million (ppm) or milligrams per liter (mg/l) - one part per million corresponds to one minute in two years or a single penny in $10,000.
Parts per billion (ppb) or micrograms per liter - one part per billion corresponds to one minute in 2,000 years, or a single penny in $10,000,000.
Nephelometric turbidity unit (NTU) - nephelometric turbidity unit is a measure of the clarity of water. Turbidity in excess of 5 NTU is just noticeable to the average person.
Action level - the concentration of a contaminant which, if exceeded, triggers treatment or other requirements which a water system must follow.
Treatment technique (TT) - (mandatory language) A treatment technique is a required process intended to reduce the level of a contaminant in drinking water.
Maximum contaminant level - (mandatory language) The "Maximum Allowed" (MCL) is the highest level of a contaminant that is allowed in drinking water. MCLs are set as close to the MCLGs as feasible using the best available treatment technology.
Maximum Contaminant Level Goal - (mandatory language) The "Goal"(MCLG) is the level of a contaminant in drinking water below which there is no known or expected risk to health. MCLG's allow for a margin of safety.

What does this mean?
As you can see by the table, our system had no violations. We're proud that your dinking water meets or exceeds all federal and state requirements. We have learned through our monitoring and testing that some constituents have been detected. The EPA has determined that your water is safe at these levels.
We test for a total of 76 contaminants. Those of which were undetected, are not included in the table. A list is available upon request.
The sources of drinking water include rivers, lakes, streams, ponds, reservoirs, springs and wells. As water travels over the surface of the land or through the ground, it can dissolve naturally occurring minerals and, in some cases, radioactive materials. The water can also pick up substances such as: Microbial contaminants, such as viruses and bacteria, which may come from sewage treatment plants, septic systems, agricultural operations and wildlife.
Inorganic contaminants, such as salts and metals, which can be naturally-occurring or result from urban storm water runoff, industrial or domestic waste water discharges, oil and gas production, mining or farming.
Microbial contaminants, such as viruses and bacteria, which may come from sewage treatment plants, septic systems, agricultural operations and wildlife.

Pesticides and herbicides, which may come from agriculture, urban storm water runoff, and residential uses.
Organic chemical contaminants, which can come from industrial processes, gas stations, urban storm water runoff and septic systems.
Radioactive contaminants, which can be naturally occurring or the result of oil and gas production and mining activities.
In order to insure that tap water is safe to drink, EPA establishes regulations, which limit the amount of certain contaminants in water provided by public water systems. The Food and Drug Administration establishes limits for contaminants in bottled water.
MCLs are set at very stringent levels. To understand the possible health effects described for many regulated constituents, a person would have to drink a half gallon of water every day at the MCL level for a lifetime to have a one-in-a-million chance of having the described health effect.
Some people may be more vulnerable to contaminants in drinking water than the general population. Immuno-compromised persons such as persons with cancer undergoing chemotherapy, persons who have undergone organ transplants, people with HIV/AIDS or other immune system disorders, some elderly, and infants can be particularly at risk from infections. These people should seek advice about drinking water from their health care providers. EPA/CDC guidelines on appropriate means to lessen the risk of infection by cryptosporidium and other microbiological contaminants are available from the Safe Drinking Water Hotline (800-426-4791) or EPA (800-227-8917).
